Accident: Hardy Bay, Near Port Hardy, BC. Lat 50.43N, Long 127.29W 24-May-1982. The floatplane pilot was evacuating a patient on a stretcher. Enroute, he encountered low ceilings and fog, so he elected to land then water taxi the remaining distance. During the prolonged taxiing in rough water, the nose compartments of the floats filled with water, and the aircraft nosed over inverted. The patient drowned before she could be released from the stretcher.

Incident: Bella Coola, BC. 14-Aug-1995. On take-off the amphibious float equipped aircraft encountered very rough water. The front strut fittings broke. The front portions of the floats were bent upwards and the bottom of the fuselage was damaged when it hit the spreader bar. The pilot was not injured. CADOR report 1995P0318.
