Accident occurred 21-Jul-2017 at Shannons Pond Seaplane Base (0Z3), Dillingham, AK.

The pilot stated that he was departing on a postmaintenance flight check in a float-equipped airplane from a short water lane at a remote, unimproved seaplane base. While taxiing for departure, he left the wing flaps in the "up" position to aid in turning the airplane on the water. After aligning the airplane for the takeoff run, he applied full power, and the airplane accelerated onto the step. While attempting to rotate, he realized that he had forgotten to reset the wing flaps to the "takeoff" position. He attempted to reset the manually operated, hydraulically actuated wing flaps, but the incorrect flap setting led to a longer-than-normal takeoff run, and the airplane subsequently impacted the water lane's far bank, which resulted in substantial damage to the left wing and left-wing strut. The pilot reported that there were no preaccident mechanical anomalies with the airplane that would have precluded normal operation.  ANC17CA038

USAF History

51-16558 was made available on 09-Sep-1952 and accepted on 15-Sep-1952 by the 6600th Air Depot AMO (Air Material Overseas Unit), at Downsview, Ontario after which it was delivered to the Tactical Air Command at Alexandria AFB, LA and the 6th Liaison Squadron on 22-Sep-1952 where it remained until 26-Mar-1953. It moved to Shaw AFB SC for a few days until 03-Apr-1953 before returning to the 6th Liaison Sqd at Alexandria This attachment was somewhat longer, until 18-Jan-1954 as it then crossed over to the 366th Fighter Bomber Wing remaining until 29-Apr-1955.

A change of assignment now saw it with the Air Training Command. The initial base was Luke AFB, AZ where it was attached to the 3600th Combat Crew Training Wing and stayed until 29-Dec-1955 when it moved to Williams AFB., Arizona joining the 3525th Pilot Training Wing which a month later on 01-Feb-1956 was re designated as 3525th Combat Crew Training (CCT) Wing.

On 07-Feb-1957 it is recorded as having returned to DHC at Downsview, Ontario remaining until 28-Mar-1957. It returned to active service back with the 3525th CCT Wing at Williams AFB. The unit re assigned to the Tactical Air Force and was re designated as the 4530th CCT Wing on 01-Jul-1958 remaining until 11-Aug-1960. Its next move was to Washington, DC., and the 1100th Air Base Wing at Bolling AFB., which was attached to Headquarters Command. It remained until 17-May-1961 after which it is shown as passing to the Oklahoma City Air Depot at Tinker AFB., Oklahoma. This was initially assigned to the Air Force Material Command and in Jan-1963 to the Air Force Logistics Command. The next attachment was to the Fairchild Aircraft Company Facility at Hagerstown, Maryland from 11-Feb-1963.

The aircraft’s further USAF history is to be researched.
