Accident: Ogden, UT. 15-Aug-1975. Whilst on dual instructional flight engine failed (bearing assembly collector and intermediate impeller-collector case eroded from rubbing, caused forced landing off airport on land. Aircraft nosed over landing on wet soft ground. Damage substantial. Two crew and two passengers sustained minor or no injuries. DEN76FTM05.

N5317G Willow Air Service, Willow, AK. Circa Sep-1985.

Accident: 1 mi. W of the summit of Merrill Pass, AK. 26-Sep-1985. The aircraft collided with terrain one mile west of the summit of Merrill Pass, AK at an elevation of 3,600 ft. mean sea level. The pilot obtained a weather briefing at 13:29 GMT on 26-Sep-1986 at which time Merrill Pass was forecast to be closed by 11:00 ADT due to deteriorating weather. The last radio communication with the aircraft was at 18:03 GMT when contact was made with McGrath FSS. At this time the pilot made a pilot report stating the weather at Merrill Pass was marginal; 2 mi. visibility with snow and occasional moderate turbulence. The wreckage was located on 29-Jun-1986 and indicated the aircraft contacted the terrain in a relatively level attitude with considerable airspeed. Pilot and two passengers sustained fatal injuries. Note: Shown on FAA records Sep-1992 as Lake Union rebuild in 1989 with a c/n 6617. This aircraft was collaboration between Lake Union Air, a Seattle based air taxi, and the Renton FAA office to produce an aircraft with a new serial number. Thus the official manufacturer of the plane is registered as deHavilland/Lake Union Air based on provisions present at the time in the FARs, which allowed the aircraft to have a zero airframe time and a new date of manufacture.

Airworthiness Date: 21-Jun-1989.

Accident: Friday Harbor, Washington. 02-Jun-1991. During a water take off with a crosswind the aircraft lifted off to a height of 15 ft., when pilot opted to abort landing. As power reduced left wing collided with a moored tugboat. SEA91LA122

However, the accident report of 02-Jun-1991 shows the aircraft to be registered as N21LU. The history with Northeast is confused as they reserve registrations and put the marks on new aircraft as they arrive to keep their sequence. E.g. N67684 which has been c/n 1207 & 1667 in recent history.
